Medical Conditions

At Bradshaw Hall Primary we are an inclusive community that aims to support and welcome pupils with medical conditions. We aim to provide all pupils, with all medical conditions, the same opportunities as others at school. If your child has any medical conditions, we ask that you complete all relevant information in the school welcome and introductory pack.

You may be asked to complete a Form 1 Individual Health Plan with the school nurse. This information will help store details about your child's medical condition, current medication, triggers, individual symptoms and emergency contact numbers.

Please keep school updated of any new conditions or changes to your child's medical arrangements.
